We can fund health insurance for every American by shifting to a two-tiered program. The first tier of basic insurance could be funded from personal income/employer benefits/Medicare/ Medicaid. The second tier could be funded by sales taxes through a partnership between states and insurance companies. Those who hold a basic plan would automatically be enrolled in the second tier with the same insurance company. Everyone wins, and everyone could have insurance.
